BACKGROUND: The temporomandibular joint (TMJ) cartilage consists of condylar cartilage and disc and undergoes continuous remodeling throughout post-natal life. To maintain the integrity of the TMJ cartilage, anti-angiogenic factors play an important role during the remodeling process. In this study, we investigated the expression of the anti-angiogenic factor, chondromodulin- 1 (ChM-1), in TMJ cartilage and evaluate its potential role in TMJ remodeling. METHODS: Eight TMJ specimens were collected from six 4-month-old Japanese white rabbits. Safranin-O staining was performed to determine proteoglycan content. ChM-1 expression in TMJ condylar cartilage and disc was determined by immunohistochemistry. Three human perforated disc tissue samples were collected for investigation of ChM-1 and vascular endothelial growth factor (VEGF) distribution in perforated TMJ disc. RESULTS: Safranin-O stained weakly in TMJ compared with tibial articular and epiphyseal cartilage. In TMJ, ChM-1 was expressed in the proliferative and hypertrophic zone of condylar cartilage and chondrocyte-like cells in the disc. No expression of ChM-1 was observed in osteoblasts and subchondral bone. ChM-1 and VEGF were both similarly expressed in perforated disc tissues. CONCLUSIONS: ChM-1 may play a role in the regulation of TMJ remodeling by preventing blood vessel invasion of the cartilage, thereby maintaining condylar cartilage and disc integrity.

Temporomandibular disorders (TMD) and psychosocial factors reportedly associate. The underlying factors remain partially obscure, however, and further studies are required to clarify the relationships. The aims of this study were thus to assess in a non-patient working population the prevalence of TMD and related symptoms, and to clinically diagnose and follow the natural courses of TMD over a one-year period. In addition, possible comorbidity of temporomandibular and/or neck muscle pain and perceived stress and their impact on work performance were investigated, as well as how various psychosocial aspects relate to TMD. A questionnaire was mailed to all 30- to 55-year-old employees of the Finnish Broadcasting Company Ltd. whose employment in the Helsinki area had lasted at least five years (n = 1784). Of the 1339 subjects, who returned the questionnaire, 241 were examined according to the RDC/TMD and standard neck muscle palpation methods. Clinical signs of temporomandibular and/or neck muscle pain were found in 118 subjects. One-year follow-up TMD examinations were conducted on 211 subjects. The prevalence of frequent painless TMJ-related symptoms was 10%, orofacial pain 7%, neck pain 38%, and headache 15%. TMD diagnoses were: myofascial pain (13%), disc displacements (16%), and arthralgia, osteoarthritis, osteoarthrosis (4%). Chronic myofascial pain was present in 7% and chronic disc displacement with reduction in 11% of the subjects. Symptoms were significantly associated with almost all the studied psychosocial symptoms. Reduced work performance was significantly positively associated with continuous pain, severity of pain, and health stress perception, and according to logistic regression, somatization with the probability of having chronic myofascial pain. It could be concluded based on the results of this study among a non-patient working population that TMD and related symptoms are common and associated with psychosocial factors. Moreover, myofascial pain and disc displacement with reduction are the most common diagnoses of TMD. In addition, self-reported health related stress, and continuous pain in temporomandibular and/or neck muscles are associated with reduced work performance, and somatization is significantly associated with chronic myofascial pain.

A articulação temporomandibular (ATM) é uma articulação complexa com características e funções únicas. Certamente é uma das articulações mais utilizadas e seu uso inadequado e excessivo consequentemente promove inúmeros transtornos. Dentre as alterações de hipermobilidade articular, o deslocamento mandibular refere-se ao posicionamento, geralmente anterior, do côndilo mandibular sobre a eminência articular, com completa separação das superfícies articulares e consequente travamento. Quando os episódios se tornam frequentes, algum método de tratamento, seja conservador ou cirúrgico, deve ser utilizado, devido ao grande transtorno funcional e social gerados. A injeção de sangue autógeno na articulação temporomandibular é uma técnica minimamente invasiva, com escassos relatos na literatura e que foi recentemente reintroduzida. Onze pacientes diagnosticados com luxação recidivante da articulação da cabeça mandibular receberam injeções bilaterais de sangue autógeno no compartimento articular superior e região pericapsular. Em acompanhamento, que variou de 24 a 35 meses (média de 29,6 meses), 3 (27,3%) pacientes apresentaram recidivas. O mesmo protocolo de tratamento foi repetido para estes três casos, porém sem sucesso, sendo então encaminhados para procedimento cirúrgico de eminectomia. Dessa forma, dos 11 pacientes inicialmente tratados, 8 (72,7%) não apresentaram episódios de deslocamento mandibular recorrente após o procedimento proposto. Trata-se de um procedimento simples, rápido, pouco invasivo, de baixo custo e com mínima possibilidade de complicações, sendo uma alternativa de tratamento viável, antes de se indicar procedimentos cirúrgicos.

The aim of this study was to conduct a systematic review to identify the randomized clinical studies that had investigated the following research question: Is the mandibular manipulation technique an effective and safe technique for the treatment of the temporomandibular joint disk displacement without reduction? the systematic search was conducted in the electronic databases: PubMed (Medical Publications), LILACS (Latin American and Caribbean Literature in Health Sciences), EMBASE (Excerpta Medica Database), PEDro (Physiotherapy Evidence Database), BBO (Brazilian Library of Odontology), CENTRAL (Library Cochrane), and SciELO (Scientific Electronic Library Online). the abstracts of presentations in physical therapy meetings were manually selected, and the articles of the ones that meet the requirements were investigated. No language restrictions were considered. Only randomized and controlled clinical studies were included. Two studies of medium quality fulfilled all the inclusion criteria. There is no sufficient evidence to support the effectiveness of the mandibular manipulation therapy, and therefore its use remains questionable. Being minimally invasive, this therapy is attractive as an initial approach, especially considering the cost of the alternative approaches. the analysis of the results suggests that additional high-quality randomized clinical trials are necessary on the topic, and they should focus on methods for data randomization and allocation, on clearly defined outcomes, on a priori calculated sample size, and on an adequate follow-up strategy.

1.Pre-assessment data of the patient A 2-year-old boy, weighing 15 kg was admitted with a history of limited mouth opening(inter-incisor distance of 6 mm), hypoplastic and retrognathic mandible (bird face deformity) and facial asymmetry from left temporomandibular joint ankylosis (TMJA). He was born at term, after an uneventful pregnancy, and there was no report of trauma during caesarean section. No other possible aetiologies were identified. He was scheduled for mandibular osteotomy. Preoperative ENT examination revealed adenotonsillar hypertrophy. 2. Anaesthetic Plan A fiberoptic nasal intubation was performed under deep inhalation anaesthesia with sevoflurane, with the patient breathing spontaneously. Midazolam (0.05 mg.kg-1) and alfentanil (0.03 mg.kg-1) were given and anaesthesia was maintained with O2/air and sevoflurane. No neuromuscular blocking agent was administered since the surgical team needed facial nerve monitoring. 3. Description of incident During surgery an accidental extubation occurred and an attempt was made to reintubate the trachea by direct laryngoscopy. Although the osteotomy was nearly completed, the vocal cords could not be visualized (Cormack-Lehane grade IV laryngoscopic view). 4. Solving the problem Re-intubation was finally accomplished with the flexible fiberscope and the procedure was concluded without any more incidents. Extubation was performed 24 hours postoperatively with the patient fully awake. After surgery mouth opening improved to inter-incisor gap of 15 mm. 5. Lessons learned and take home message Two airways issues present in this case can lead to difficultventilation and intubation: TMJA and adenotonsillar hypertrophy. These difficulties were anticipated and managed accordingly. The accidental extubation brought to our attention the fact that, even after surgical correction, this airway remains challenging. Even with intensive jaw stretchingexercises there is a high incidence of re-ankylosis, especially in younger patients. One should bear that in mind when anaesthetizing patients with TMJA.

La enuresis es un trastorno frecuente en niños, afecta el desempeño social y familiar. Se desconoce en nuestro medio la relación entre enuresis, trastornos del comportamiento y disfunción familiar. Objetivo: Determinar diferencias entre un grupo de niños con enuresis y un grupo control, atendidos en la Clínica Infantil Colsubsidio en la frecuencia de disfunción familiar y trastornos del comportamiento a través de cuestionarios diligenciados por las madres. Materiales y métodos: Diseño Corte Transversal con componente analítico. Se formaron 2 grupos: niños con enuresis y un grupo comparable sin enuresis. Se realizó entrevista a las madres quienes diligenciaron el cuestionario apgar familiar y de capacidades y dificultades. Se compararon los puntajes obtenidos en ambos grupos. Resultados: El grupo de pacientes con enuresis mostró disfunción familiar con mayor frecuencia que en el grupo control (P = 0,005). Las variables de comportamiento se dividieron en 5 categorías: síntomas emocionales, problemas de conducta, hiperactividad, problemas en relación con los pares y conducta prosocial. Se encontraron puntajes totales anormales con mayor frecuencia en el grupo con enuresis (P < 0,001). Ambos grupos tuvieron diferencias significativas para cada una de las 4 primeras categorías. Conclusiones: Los pacientes con enuresis presentan con mayor frecuencia disfunción familiar y trastornos del comportamiento. El diseño del presente estudio no permite establecer relaciones de causalidad, pero sí recomendar incluir en el estudio de paciente con Enuresis una evaluación de la funcionalidad familiar y comportamiento de los niños además de los estudios dirigidos a descartar trastornos orgánicos relacionados.

Objetivo: establecer la prevalencia de la disfunción sexual en las pacientes sometidas a corrección de incontinencia urinaria por medio de la uretrocistopexia con cabestrillo en el Hospital Universitario Mayor. Metodología: se realizo un estudio analítico de corte transversal donde se evaluara la disfunción sexual en pacientes con incontinencia urinaria por medio de la encuesta PISQ-12 después de 6 meses de la realización de cabestrillo suburetral dentro del manejo de incontinencia urinaria femenina y buscarán la asociación con otros factores como el tipo de cirugía asociada al cabestrillo, menopausia, terapia hormonal, edad, número de embarazos utilizando la prueba de asociación ji-cuadrado de Pearson o el test exacto de Fisher o razón de verosimilitud exacta (valores esperados < 5). Resultados: la prevalencia de disfunción sexual fue del 27% (12 pacientes), de ellas 25% tuvieron disfunción moderada (11 pacientes) y dos por ciento disfunción severa (1 paciente) deacuerdo con la escala PISQ-12. La disfunción sexual fue más frecuente en las pacientes con prolapso posterior estadio 2 (4 de las 5 mujeres), seguido del prolapso anterior y posterior estadio 2 (4 de 10 mujeres), las otras categorías fueron menores, mostrando asociación significativa (p=0.011, Test exacto de Razón de verosimilitud). Conclusión: del presente estudio podemos concluir que la cirugía de piso pelvico (colporrafías) contomintante a la cirugía de incontinencia urinaria con cabestrillo suburetral está asociada a un mayor grado de disfunción sexual femenina.
